Schedule F - Contribution to the health services fund (Rama please add French for entire page)

The purpose of this contribution is to ensure that everyone contributes to the financing of Québec’s health services. Consequently, you may have to pay a contribution to the health services fund if you were resident in Québec on December 31, 2016 (or on the date you ceased to be resident in Canada in 2016), and your total income (line 199), minus the total of the amounts on lines 101 and 105, is over $14,440. To find out if you are required to pay a contribution and to calculate the amount thereof, complete Schedule F.

The value in all the lines in Schedule F part A flows automatically from TP-1  return and the contribution amount is computed automatically in Part B which flows to Line 446 of TP-1 return.
